Personal Health Assessment (PHA) Questionnaires need to be completed by October 31! Please take a few minutes to go online and complete the questionnaire. Once you’ve finished the questionnaire, you’ll immediately get your full PHA report. This report summarizes both your lifestyle habits and your biometric score (from your lab-work, blood pressure, height and weight measurements) and provides individualized recommendations for improving your health. Please email (laurie.ehlers@lawrence.edu) or call (832-7498) if you’d like to set up an appointment to review your report in person, or to discuss strategies for implementing any of the recommendations.
