Personal Health Assessment (PHA) Registration is Now Open

What is a PHA?

The Personal Health Assessment is a tool that highlights how our lifestyle choices directly impact overall health. Two scores are provided:

  • Your Lifestyle Rating is based on your self-reported daily habits related to nutrition, physical activity, alcohol consumption, stress and depression, and tobacco use.
  • Your Biometric Score is based on tobacco use, Body Mass Index (BMI), blood pressure, cholesterol levels, and blood sugar.

The Personal Health Assessment Report provides personalized feedback, tips to improve your lifestyle habits, and resources for more information.

Who is eligible to participate?

All staff and faculty are welcomed and encouraged to participate in the PHA process. Participation is also encouraged for the following individuals covered under the Lawrence University medical plan: spouses/domestic partners of employees, retirees and spouses/domestic partners of retirees. 

Note for Medical Plan Participants

Participation by employees, retirees covered under the medical plan, and spouses/domestic partners covered under the medical plan will provide the discounts to the medical plan premium.  All participants must complete both the biometric screening and the lifestyle questionnaire to receive the premium discount. The questionnaire can be completed through Wellworks portal (ThedaCare’s new platform) while signing up for your biometric screening appointment.  Discounts are provided based upon participation and are not scoring or results based.

Along with their new platform, Wellworks, ThedaCare will be switching to a finger stick blood collection as opposed to an arm draw. This process is less invasive and gives instant results.

If you or your spouse/domestic partner is pregnant or within 6 months post-partum during the time of the assessments, participation by the pregnant/post-partum individual will not be required to obtain the discounts on medical plan premiums. Human Resources must be notified if this is applicable to you or spouse/domestic partner by October 15, 2021.  

How do I sign up?

Scheduling directions can be found in the 2021 PHA Instruction Sheet or click here to sign up: www.wellworksforyoulogin.com. The PHA schedule books quickly.  To ensure you receive your preferred appointment time, do not delay in scheduling your appointment.

What is the PHA schedule?

All appointments will be held in the Warch Campus Center – Hurvis & Mead Witter rooms. Appointment time and dates are as followed:

  • Wednesday, October 6th at 6:00am – 10:30am
  • Thursday, October 7th at 6:00am – 10:30am
  • Friday, October 8th at 6:00am – 10:30am
  • Monday, October 11th at 6:00am – 10:00am
  • Tuesday, October 12th at 6:00am – 10:00am
  • Wednesday, October 13th at 6:00am – 10:00am
  • Thursday, October 14th at 6:00am – 10:00am
  • Friday, October 15th at 6:00am – 10:00am

  • Succeed at Work: Increasing Mental Toughness – September 16th from 12:00 – 1:00pm
    • Explores how to use challenging times as an opportunity to increase mental toughness and find a positive way to look at negative events. Participants will discuss what is means to be mentally tough and will also dive into the topic of “Grit.”
  • Strong Parents, Strong Families: Encouraging Effective Homework & Study Habits – September 22nd from 12:00 – 1:00pm
    • The discussion will include: the role of parents/guardians, helping children with homework, tutoring options, working with teacher(s), and encouraging learning outside of school.
  • Your Healthy Lifestyle: Promote Family Health – September 23rd from 12:00 – 1:00pm
    • This talk is designed to empower people with knowledge to promote healthy living on the home front. We will discuss the benefits of healthy living, nutrition and exercise, doctor’s involvement, and how to have informed conversations with children.

This process is less invasive and gives instant results.

Upon completion of the PHA, participants will receive a personalized report to help better understand how your lifestyle habits affect your overall health.  Participants will be able to log in to the Wellworks site (new ThedaCare platform) to complete the questionnaire, sign up for their onsite screening, and to view their results. Your results of the biometric and lifestyle scoring are confidential.  Participation in the PHA process is open to all staff and faculty employees and all spouses/domestic partners on the Lawrence University medical plan.

    • This month’s live talk offers participants a virtual home makeover tour of the latest product designs and modifications to make homes work at any age. Multigenerational living is on the rise and a Livable Home Maintenance Plan will be provided to guide smart and green home living.
  • Succeed at Work: The Science of Goal Setting – August 19th from 12:00 – 1:00pm
    • Examines goal setting as a brain function. Participants will look at how the brain works and create an awareness of the mental process, making it easier to develop new habits in relation to goal setting.
  • Strong Parents, Strong Families: Children’s Nutrition – August 25th from 12:00 – 1:00pm
    • Participants will learn the basics of nutritional requirements for growing children, tips for healthy eating, health risks of obesity in children, recognizing when weight is a problem and building self-esteem. 
  • Your Healthy Lifestyle: Healthy Meal Planning – August 26th from 12:00 – 1:00pm
    • Learn how to feed your family healthy foods on a budget, how to identify healthy foods and ingredients, and how to incorporate them into your meals. This talk will help you identify ways to increase your intake of fruits and vegetables, understand portion control, and learn to read and understand food labels.

Due to COVID-19, a Transamerica Retirement Planning Consultant is unable to come to campus. In lieu of in-person meetings, they are offering individual virtual appointments during a variety of shifts until June 30th.
